1893. NEW ZEALAND.

HOSPITALS AND CHARITABLE INSTITUTIONS IN THE COLONY. (REPORT ON THE), BY THE INSPECTOR OF HOSPITALS.

Presented to both Houses of the General Assembly by Command of His Excellency.

The Inspectoe of Hospitals to the Hon. the Ministee of Education. Sic, —. I have the honour to lay before you the following report on the charitable institutions and hospitals of the colony : — Last year I dealt fully with the weak points of our Charitable Aid Act. Nothing of importance has occurred this year to alter the position, except a fresh outbreak of the difficulty between the Wellington United District Board and the Wellington Benevolent Society. After careful inquiry and observation, I am convinced that the difficulty is insuperable under the existing law. Some means must be found for overcoming, or, at any rate, mitigating the difficulty of tying together for rating purposes the country districts and the large centres to which the sick and poor naturally gravitate. The defective provisions of the Act, as to what "maintenance " of hospital buildings legally means, combined with the large grant made to the Dunedin Hospital, have had the effect of bringing before the Government such large demands for new hospital-buildings in other places, that very grave difficulty- has been experienced in dealing with these claims. A change in the law is urgently needed to deal with this matter. The total expenditure for hospital and charitable aid for the year has been £153,955 16s. Bd., showing an increase of £9,014 19s. lid. over that of the previous year. This increase is owing to the founding of the following new institutions : The Palmerston North Hospital, at a cost of £2,456, of which £2,282 18s. was for new buildings; the Jubilee Institute for the Blind, Auckland, at a cost of £3,780 ; the Jubilee Home, Wanganui, £890; and the Benevolent Home, Wellington, at a cost of £2,555. All these institutions were urgently required. The last, especially, has greatly relieved the difficulties of the Wellington Benevolent Society, though, owing to the delay in comprehensive legislation dealing with this subject, the trustees were obliged to build in a position which cannot long meet the requirements of the district. lam bound, however, to say that they have made the best use of the means at their disposal. I made a careful inspection of the Homes and Befuges maintained under the Act, and found that they were all carefully looked after by the local authorities. The difficulties of providing new buildings under the Act have here, as in the case of the hospitals, been found so great as to still cause great embarrassment, especially at Napier and Taranaki.

This hospital contained only two patients on the date of my visit, the 25th December. I found everything in good order. In some of the smaller towns I find it almost impossible to obtain supplies for the hospital without infringing the terms of " The Local Bodies Contractors Act, 1885." In consequence of a dispute arising out of this, Dr. Donaldson left Arrowtown, and Dr. Nichols, formerly of Waimate, was appointed in his place.

This hospital has had a quiet and prosperous year. Great improvements have been effected in the ventilation of the wards. There has been such an increase in the number of patients that the trustees are taking steps to increase the accommodation, and have approached the Government for a grant for that purpose.

The agitation which prevailed some months ago about the insanitary condition of some of the old portions of this building seems to have died away. At any rate, at my last visit, I found wards No. 1 and No. 2 quite fresh and well ventilated. The new nurses' home will, I hope, soon be put in hand, and the nursing system, which has for long been the weak spot of this hospital, will then be reorganized and improved. The two new wards added to this hospital are the finest in the colony. One of the old wards ought to be utilised for a children's ward. The country ought not to be asked to provide any more accommodation at this institution for a long time to come. The money already granted by the Government has caused such a number of demands for additional accommodation in other places as to make it quite impossible to satisfy them. The new matron is efficient, and is, I believe, giving universal satisfaction. The institution, as a whole, as well as the medical school attached to it, deserve the confidence of the public.

All the objections which I had previously made to the ventilation of this hospital have been carefully remedied, and I found the fullest evidence of careful management on the part of all concerned.

PALMEBSTON NOBTH HOSPITAL. Palmerston North, 7th March, 1893. This is a commodious and well-designed new hospital, which has not yet been opened because of some delay in getting the drainage properly arranged. A great deal of interest has been shown in raising money locally for the building. A sum of £1,382 12s. 2d. was raised by local subscriptions, and the total cost up to date has been £2,282 18s.

The event of the year in connection with this hospital has been the decision to erect a fine new wing to meet the growing requirements of the city. The Government have largely assisted, under pressure of a typhoid epidemic. The management in all the departments is such that the hospital is too popular in the town, in the sense that a great many people are tempted to go there for treatment who may be able to pay private practitioners. The out-patient department ought not to be encouraged to assume such large proportions.

The whole administration of the Thermal Springs Act — i.e., the management of the lands, the control and development of the water-supply, and the construction and care of the baths—fall to the Lands Department. We have charge only of the hospital, and the medicinal use of the baths. The principle which guides the Government in managing this institution is that of avoiding all competition with private enterprise so far as providing accommodation for the general public is concerned. To that end admission to the hospital is restricted to those persons who are not able to afford hotel expenses, and who are therefore unable to take advantage of the wonderful curative properties of the waters without public assistance. This assistance is given according to well-known rules, which have been published in the Gazette. The institution was inspected by me in October last, and I found everything in a satisfactory condition. The total number of patients treated during the year was 59; the average number resident during the year 8-75. The daily average cost per head was Bs. 3f d. ; less amounts received for maintenance, ss. 5Jd. It must, however, be pointed out that the doctor's salary has to be relatively much larger here than in any other hospital in the country—viz., £400— owing to the difficulty of inducing a good man to remain in such an isolated place all the year round, whose services should be available for the general public and for the Natives. Besides this, a large sum, viz., £202 per annum, has to be paid to bath-attendants, whose services are largely required to attend on invalids and other bathers who reside at the various hotels and boarding-houses. It is impossible therefore accurately to determine what proportion of these expenses should be charged against the hospital, which moreover does not receive the bath-fees. I can say for certain, however, that considering the high prices which have to be paid for everything in this remote and unproductive district, the management will compare favourably in regard to cost with that of any other hospital in the colony.
