TRAIN DISASTER
Personnel of Board of Inquiry THE ORDER OF REFERENCE (NZ.P.A.) WELLINGTON. Nov. 'l. The Minister of Railways (the Hon. R. Semple) announced to-day that a Board of Inquiry had been set up to investigate the circumstances of the derailment of the passenger train near Havwards. Hutt Valley, on Monday. The Board, which consists of Sir Francis Frazer (chairman), Mr John Wood and Mr Harold Lintercale. is to report to the Minister by December 10. The order of reference fo the Board 1S ’(1) What was the cause of the deailment? (2) Was any person in the employ of the Government Railways Department guilty of any direliction of duty which directly or indirectly contributed to the derailment? (3) Generally, any circumstances in connection with the derailment which call for comment, particularly in the respect of rolling stock and permanent way in the locality of the derailment. (4) What steps, if any. should be taken to prevent the occurrence of a similar derailment?
