A FATAL ERROR
How a tube was found to be wrong'■}y connected" to an anaesthetic appar•atus used on a patient in. the operating, theatre at the Chesterfield Royal Hospital was described at the inquest 'on Edna May Souter, aged eighteen. ;A doctor, who-administered the anaes-. thetic. consisting of gas and 'oxygen, •Said that the gas started to give out .and he resorted to ether vapour. When he noticed'that one .of the tubes'had been wrongly connected by' a nurse the patient was given, an injection to bring her back to semi-conscious-ness. ; ,\The: operation was not performed.Dr. Kilpatrick gave evidence •that Miss Souter died two days later from- bronchial pneumonia brought on by the irritation of the ether. A verdict of "death by misadventure", was returned, the Coroner observing that a committee of the hospital staff .was taking the matter up. . ■ '
