FORMS REQUIRED Form E.P. 32 (six-monthly form) is now due, and must be returned to the Employment Bureau by all men on scheme 5 not later than Friday, April 23. Levy books must be presented with the completed form. Any man failing to return his form by the above date will be deleted from his group and will not receive any further relief until he has been re-posted by the Employment Bureau.
