HOSPITALS AND CHARITABLE INSTITUTIONS OF THE COLONY (REPORT ON THE), BY THE INSPECTOR-GENERAL.
Presented to both Houses of the General Assembly by Command of His Excellency.
The Inspector-General op Hospitals and Charitable Institutions to the Hon. the Minister for Public Works. 2nd July, 1903. The total expenditure on charitable aid in New Zealand in 1901 was £79,873; in 1902, £88,848; for the year ending the 31st March, 1903, it amounts to £93,158. The amount distributed in outdoor relief for the year ending the 31st March, 1902, was £38,934; for the year ending the 31st March, 1903, it amounts to £43,421. In the four larger towns the expenditure on outdoor relief alone is as follows : Auckland Charitable Aid Board, £4,923 ; Christchurch Charitable Aid Board, £7,356 ; Dunedin Benevolent Trust, £6,048 ; Wellington Benevolent Trust, £4,685. It will be seen that there is considerable increase in outdoor relief all along the line. The Wellington Benevolent Trust has increased its outdoor relief during the past year by £1,305. It is a fact accepted by all careful students of social science that pauperism grows by giving. Too-freely administered outdoor relief merely intensifies the evil its administrators desire to mitigate. Of the charitable institutions for old and destitute persons, the Ohiro Home, Wellington, the Jubilee Home, Christchurch, and Tuarangi, Ashburton, are the most satisfactory: the food, clothing, cleanliness, and comfort of the inmates all that could be wished. At the Jubilee Home everything possible is done for the comfort and well-being of the old folk by the indefatigable Matron, Miss Higgins, and the Charitable Aid Board spends ungrudgingly whatever is necessary to keep the Jubilee Home at a high standard of comfort. At the Ohiro Home Mr. and Mrs. Purvis are very successful managers, and the transformation in food, clothing, and comfort effected here within the last few years is admirable. At Tuarangi Mr. and Mrs. Morrissey have only old-men inmates —no women —and they are kind and judicious in their management. Lome Farm, near Invercargill, does not quite come under the category of the above-mentioned institutions, as it is not solely for the benefit of the aged. Mr. and Mrs. Cusworth have under their charge thirty-four children, and also some twenty old men, who are able to do a little farming and gardening. This institution continues to deserve praise, and is a credit to its managers and to the Southland Charitable Aid Board. At the Old Men's Home, Invercargill, the decrepit, bedridden, and feeble aged are carefully looked after by Mr. and Mrs. Cole. The Benevolent Institution, Caversham, is still overfull and understaffed. Mr. and Mrs. Mcc have about 270 inmates under their care, a large number of these being bedridden, paralysed, or enfeebled with senility, and yet the Master and Matron have no officers to assist them, excepting for a succession of nurses (unpaid) who come for three-month periods to assist with maternity cases. Many years ago, before the new buildings began to be erected, I did my best to impress upon the Trustees how unwise it was to erect new buildings on so small a piece of ground, amounting to not more than eight acres and a half. Now the time has come when even the blind can see that a great mistake has been made in placing so many expensive buildings on so small a site. A piece of good land must be obtained somewhere in the Taieri, and a building erected to be worked in connection with the central institution. The Costley Home, Auckland, is in much the same condition as the previous year, the only change being that Miss Mills has replaced Miss Phillips as Matron. The structural changes which I recommended on my visit in September had not been carried out on my visit in the following March. New beds are needed all through, I—H. 22.

There is nothing fresh to report about the Home for the Aged Needy, Wellington ; Mr. and Mrs. Mackenzie are very kind to the old people. The Nelson and Timaru Old Men's Homes continue to be carried on in the same satisfactory way. Of the Oamaru Home there is> nothing fresh to report. The Thames and Whangarei Homes are both managed very satisfactorily. For many years I have endeavoured to persuade the Board to remove the Napier Old Men's Home on to a good piece of land in the country. In the time of Mr. Ballance they could have had the former Immigration Barracks, where the soil was good, and the buildings could easily have been made suitable ; but, for some reason or other, the Board could not be induced to take advantage of this opportunity. The present site, as I have often pointed out, is far too limited in size; but its undoubtedly cheerful a:nd beautiful situation seems to have weighed with the majority of the Board, so that nothing has hitherto been done but to patch up the insect-infected wood and the present building is kept as clean as may be. The Samaritan Home, Christchurch, still has its doors open for women of the vagrant and destitute class, and about twenty old men ; it also gives shelter and tendance in maternity cases. Christchurch has more refuges, homes, and recipients of aid in some form or other than most towns. Whether this does not intensify the poverty by leading the poor to apply for help at the very first breath of trouble, instead of putting their shoulders to the wheel and courageously preserving their independence, is an open question. Any way, it may be noted that, notwithstanding the multiplicity of Church and charitable agencies, outdoor relief is distributed in Christchurch to a very much larger extent than in other towns. The Linwood Eefuge, Christchurch, and the Alexandra Home, Wellington, are doing kindly work on behalf of young mothers. The good influence of Mrs. Scott, the President of the Eefuge, is felt long after the girls and babies have left the institution. The Alexandra Home is cleaner and brighter than formerly, but there is crying need for further improvement in the dryingroom and laundry. The Levin Memorial Cottage Home is a model institution of its kind; the twenty-five little girls are thoroughly well "mothered" by Miss Ockenden. The Convalescent Home, Wellington, has lost its admirable Matron, Mrs. Butter: her successor is Miss Oakley, and she will doubtless carry on the policy of economy combined with comfort and kindliness that has hitherto made this Home an example of wise management. A considerable difference is shown in the amount spent on "rations" and on "wines and spirits" in hospitals of about the same size. Take Timaru, Thames, Napier, Invercargill, and Wanganui:—
The hospitals on the West Coast must be classed by themselves, and considered as two-thirds homes for old men and only one-third as legitimate hospital cases : —
The item "wines and spirits" for Auckland Hospital amounts to £314 for the year; Christchurch Hospital, £61; Dunedin Hospital, £186 ; Wellington Hospital, £173. As a whole our hospitals are very creditable institutions, and year by year it becomes less necessary to find fault. The Nurses Registration Act has now been in force for a year and a half, and is working smoothly. There are 320 names of nurses on the register. Twenty-nine nurses entered as candidates for the examination held in May last, and twenty-seven of these passed and are duly registered. Such an Act is necessary owing to the advance of scientific surgery and progress of medical knowledge. It protects medical men, and also the public, from the increasing number of unqualified women who pose as nurses, and who too often prove a real danger in critical illness and in operations. It is interesting to note that within the last few months five of the American States—North Carolina, New York, Illinois, Virginia, and New Jersey—have followed the lead of this colony, and passed Acts for the State registration of duly qualified nurses. The Nurses Registration Act of New Zealand uses no compulsion, except that of enlightened self-interest on the part of the nurses themselves, and it is becoming daily more apparent that by the silent pressure of this law the nursing profession of New Zealand will be effectively organized. The whole working of it has been intrusted to Mrs. Grace Neiil, and the tact and skill by which she has carried out all the

arrangements, often complicated and difficult, as regards the examinations, have earned my highest appreciation and gratitude. These bi-annual examinations have been moulded on the system of the New Zealand University. The examiners have no knowledge of the name nor hospital of the candidates whose papers they are examining, "and great care has been taken in selecting as examiners medical men who have had special knowledge of the training of nurses. The examination in practical work of each nurse is undertaken by a medical man and a registered nurse jointly.

The hospital-work, medical and surgical, is extremely well carried out by Dr. Anderson. Here, and at other hospitals, I pointed out that it was undesirable to engage any but trained nurses, for the facilities to do justice to probationers in the matter of training do not exist.

He has been the heart and soul of the administration of this Hospital for many years, and a more efficient hospital-manager could not have been found. The sudden death on the 24th May of Dr. Browne, the senior Eesident Medical Officer, caused grief throughout the Hospital. He had proved himself an able and conscientious officer, was well liked by all, and his death was felt to be a real loss to Dunedin. I found all departments working smoothly, and the Medical School proves an increasingly powerful stimulus to medical and surgical work in the wards.

Dr. Morrison is the Medical Officer, and takes great interest in his work. The management and working of Gisborne Hospital is so satisfactory that it is hoped the Hospital Trustees will hold on their way, and not consent to the change of method urged by some of the local medical men.

Under the care of Drs. Morice, senior and junior, this Hospital has always been, so far as their work is concerned, admirably managed. The difficulty that has always hampered the Trustees has been the large number of chronic cases. This has made the Greymouth Hospital more of an old men's home than a hospital properly so called. This will necessarily continue to be the characteristic of all the West Coast Hospitals until such time as the people of Westland determine to have an old men's home somewhere in the neighbourhood of Greymouth, the natural centre of all means of communication on the Coast. Westport, though isolated by land except via Reefton, is still easily accessible by sea.

Visited 15th October.—Considerable improvement has been effected at this Hospital during the year. The new operating-room has been furnished with the latest fittings out of money raised locally. The Hospital has had a bequest of £2,000 from the late Mr. Melmore, but the Trustees have not yet decided how this is to be used. Detached accommodation is to be provided for the nurses, and a start has already been made. This year Hawera has been made a separate hospital district, having formerly been administered by the New Plymouth Hospital and Charitable Aid Board.

To illustrate how the West Coast hospitals are hampered with chronic cases, the female ward, containing nine beds, had seven beds occupied by chronics. I inspected the new hospital-site, and though not, in my opinion, equal to the one I formerly approved, still it will be a good site for a hospital when connected with local drainage and watersupply. It consists of sor 6 acres of good land resting on a shingle subsoil.

The nurses' new quarters are comfortable and convenient. It is a pity that the ward for infectious cases could not have been built at a greater distance from the main building.
WAIKATO HOSPITAL. Failed to furnish returns. Visited 21st March.—This Hospital continues to be well administered in every respect. The linoleum on the floors has been greatly and, I think, needlessly destroyed by the method of moving the beds on to the verandahs. The books of the Hospital are not properly kept, and the annual returns have not been sent to the Department.

Dr. Moore has been appointed Medical Officer. The site for the new Hospital is a good one, on a slight rise near the town. The timber was lying on the ground, but the erection of the building had not started.

The nursing is well looked after, and it is a burning question in the district as to whether the nurses shall be accommodated in the new ward divided into cubicles, or should have a separate building to themselves. I recommend the latter course, and think that £1,500 or £2,000 ought to provide adequate accommodation.

It is not right that stretchers should be introduced into the wards at night, over and above the number of filled beds. However, a start has at last been made with the building of the Nurses' Home, and when that is ready for occupation two large wards will be set free for the use of patients. During the year Miss Payne resigned from the matronship, upon her appointment to a similar post at Rotorua Sanatorium. Miss Payne's resignation was a great loss to the Hospital. She was a good administrator, and with Dr. Ewart had done admirable work in advancing and improving the nursing department. Miss Pettitt has succeeded Miss Payne as Matron. The Wellington out-patient department entails heavy work on the Eesident Medical Officer, and the appointment of Dr. Elliott as Assistant Medical Officer will, I hope, relieve Dr. Ewart. The individual cases last year numbered 2,804, with 14,046 attendances. Dunedin Hospital comes next with 1,748 individual cases and 4,710 attendances.
