1885. NEW ZEALAND.

HOSPITALS IN NEW ZEALAND, (REPORT ON), BY DR. GRABHAM, INSPECTOR-GENERAL OF HOSPITALS AND ASYLUMS.

Presented to both Houses of the General Assembly by Command of His Excellency.

The Inspector of Hospitals to the Hon. the Colonial Secbetary. Bib,— Wellington, Ist June, 1885. I have the honour to present the following report for the year 1884. No alteration has occurred in the number of the colonial hospitals, which remains at 38. All of these, with one exception, have been visited at least once since my last report, the more important ones twice, or, in some cases, more frequently. Charleston Hospital, being unoccupied when I was at Westport, was not inspected. Alterations and Additions. Government expenditure on new buildings and enlargements has been almost in abeyance, pending improved legislation, during the past financial year. I have, however, to note the following works which have been carried out or completed since the date of my former report:— Auckland. —A hot-air disinfecting chamber has been erected. Hokitika. —Two small wards built for purposes of isolation or separate treatment. Invercargill.— Kitchen enlarged, laundry built, also a patients' dining-room, linen-store, a ward for four patients, and three bedrooms for officials. Other improvements of existing buildings have been effected. New Plymoitth. —Better rooms provided for Steward and Matron, their former apartments being thrown into the hospital, thereby adding to the patients' accommodation. Napier. —The new children's ward, and ward for females completed, furnished, and fully occupied.. Kitchen enlarged. Boss. —A new surgery and dispensary provided in a detached building. Botorua. —Hospital buildings further advanced towards completion. Thames. —A large new surgical ward, operating-room, and a small isolating ward erected. Wakatipu. —A large ward for female patients built. A number of minor improvements are noticed in the individual reports of the hospitals. Condition op Hospitals. The improvement alluded to last year continues. As a rule the establishments are kept scrupulously clean and in good sanitary condition. Additions in the way of comfortable bedding, furniture, and ornamental objects are constantly being made. Nursing. 1 have no alteration to report. Wellington continues to furnish the only training-school for probationers. Auckland would follow suit, but has no proper rooms"for the purpose. Christchurch and Dunedin might well improve upon their old-fashioned arrangements.

Distribution of Hospitals. In several instances the chief reason for providing a hospital has been the desire to have a resident medical practitioner, for whom no sufficient inducement offered in the absence of a Government subsidy. This is, of course, a very extravagant mode of subsidizing a surgeon; and the salary paid to him is often quite disproportionate to the work which he is called upon to perform. I am still of opinion that several of the hospitals might be at once closed, without inflicting any great hardship; and that others might be so modified as to form branch establishments, or cottagehospitals, attached to other more important establishments in their neighbourhood, and placed under the same management. Considerable saving of expenditure should result from such a step. On the other hand, there are large districts of the colony at present destitute of any hospital accommodation within reasonable reach. Ag population increases new establishments will be needed. Maintenance of Hospitals. I would again express my opinion that the present want of any proper system of subsidizing is fast doing away with any local efforts to raise funds. In many places charitable contributions have entirely ceased. The provision of practically gratuitous hospital treatment by the central Government is pauperizing the population to an alarming extent, and taking away inducements for thrift. Extravagance in the management prevails in the hospitals, and will continue to do so while the locality benefits by the expenditure of Government money. Patients. A large proportion of the patients treated in the hospitals are not proper objects of charity' Legislation regarding collection of maintenance-money is defective, and the machinery inefficientPayment, on recovery of health and income from employment, should be insisted on. The hospitals are encumbered with aged persons, incurable cases, and worthless persons, who would find a home in a workhouse in England. The cost of maintaining these people in a hospital far exceeds what would be necessary in a properly-appointed refuge. It will be seen from the annexed tables that, at the end of the year 1884, forty-seven more beds were in occupation than at its commencement. On the Ist January, 1884, there were 643 in-patients in the hospitals. During the year 5,625 new cases were admitted, making a total under treatment of 6,268. Of these, 5,019 were discharged and 559 died, leaving on the 31st December 690 still under care. Accommodation. The increase above referred to is by no means met by the new buildings and additions which have been carried out. The pressure for room has been chiefly felt at Auckland and Wellington; an increase of 17 patients having occurred at the former hospital and of 13 at the latter. Enlargement is badly needed at Auckland, and would best take the form of a fever-hospital and suitable buildings for the accommodation of nurses and probationers. The hospital is over-crowded with bond fide patients, and the few refuge cases to be found there inhabit rooms in the basement little suited for hospital purposes. At Wellington the removal of the aged and incurable would release a number of beds for their legitimate use. An additional fever-ward is required, so that both sexes may be treated simultaneously ; rooms are also required for the Lady Superintendent and nurses, who now occupy the only small wards which could be used for special or separate treatment. At Christchurch there is ample accommodation, while at Dunedin one large ward is usually empty and undergoing cleansing and renovation. Eeceipts. Subscriptions and donations amounted to £7,208 17s. 9d,, as against £9,269 9s. 7d. in the previous year. The amount paid by patients for maintenance was £3,964 15s. Id., whereas in the year 1883, £4,454 4s. 6d. was received under this heading. The tables both of receipts and expenditure are very imperfect. Some hospitals have furnished no information whatever, while others send a return improperly classified. A uniform system of book-keeping should be introduced. ExPENDITUKE. The total expenditure during the year 1884 amounted to £67,825 18s. 4d., or nearly £4,000 more than in the previous year. The returns furnished to me are so imperfect that I can offer no comments on the various items, but may notice that the cost of alcoholic liquors appears to have diminished to some extent. This decrease may be more apparent than real, no details being supplied regarding the hospitals at Greytown, Masterton, Greymouth, Queenstown, Eoss, and Wellington. It is quite safe to assume that a much smaller consumption would meet the real wants of the patients. I recently noticed a patient, said to be suffering from incurable disease, to be taking wines and spirits to the value of some thirty-two shillings daily, at contract prices.

Begisteks and Books. A new register has been brought into use throughout the colonial hospitals, and I have satisfied myself by inspection that it is in every case properly kept. There is no uniformity regarding other books. Case-books should be the rule, and not the rare exception, as at present. Visits of Inspection. My visits to hospitals during the year 1884 numbered 72, and I travelled 6,452 miles while inspecting these institutions and the asylums.
