1905. NEW ZEALAND.

HOSPITALS AND CHARITABLE INSTITUTIONS OF THE COLONY (REPORT ON THE), BY THE INSPECTOR OF HOSPITALS.

Presented to both Houses of the General Assembly by Command of His Excellency.

The Inspector of Hospitals and Charitable Institutions to the Hon. the Minister for Public Works. Sir, — Ist July, 1905. The Act under which this Department has been working since 1885 was a compromise between the centralising tendency to strengthen the General Government and the provincial spirit which had previously been the chief factor of the national development. When the Hospitals and Charitable Aid Act was before the House, the forces which made for local jontrol of our public institutions were still so strong that any harmonious adjustment of central control, and the demand for local administration, was impossible. When the provinces were abolished, no attempt had been made to organize our system of local government to fit it for the exercise of the functions which must devolve upon it. The chief objects aimed at by our Act were to limit by creating large districts the tendency to needlessly multiply separate institutions, and to secure equality and uniformity of taxation for buildings and maintenance by direct taxation. The latter of these objects was successfully attained so far as to cast half the cost on the rates. An attempt was made to prevent the killing of charity by giving special power to elect trustees, and a subsidy of 24 shillings in the pound for voluntary subscriptions ; but this has disastrously failed in its purpose, and the power to elect trustees has in some places been an unmitigated evil. Except in times of spasmodic activity, the subscribers dwindled and became indifferent, and their meetings were hole-and-corner conferences of self-seekers to run the institutions. If ever their seats were in danger they were always able to get back by getting elected to represent some of the outlying local bodies. The results of this can only be surmised. The determination of our people, so characteristic of our race, to have control of the spending of the money raised by taxation in their own districts has completely broken down the first of the two main objects of the law. Every year, in spite of my incessant efforts to prevent the undue multiplication of hospitals and other institutional foci of charity, vicarious because raised by taxation, their number goes on increasing. Last year our hospitals and charitable aid expenditure was £259,615, and our total population is 908,114. Outdoor relief ought no longer to be subsidised by more than a third, and abolished by a date to be fixed. Our large hospitals should be made to depend on direct taxes and local contributions, and our smaller hospitals ought to be closed as the means of communication make it possible. Only cottage hospitals should be maintained in many places where fully equipped modern institutions are aimed at. These cottages should be centres for the operations of certificated nurses who are also qualified midwives, and they could have trained helpers sent from the centres when bad cases turned up which could not be sent at once to the larger hospitals and when night nursing would be necessary. Comparatively few properly equipped hospitals could overtake all the most important hospital cases, and the others should be closed without hesitation,

Charitable Institutions for Indoor Relief. The Old People's Home, built out of the money left by the late Mr. Costley, of Auckland, has been greatly improved by providing a new kitchen block, and refurnishing some of the wards. The former master, Mr. Skinner, has resigned since my last visit, and the new arrangements have not yet had a sufficient trial to test their success. The Old People's Home, New Plymouth, the Ohiro Home, Wellington, the Jubilee Home, Christchurch, the Ashburton Home, and the Lome Farm, Southland, stand by themselves as well managed comfortable homes for the poor. The Benevolent Institution, Caversham, near Dunedin, has now outgrown both its site and its organization, which worked well for many years. I have often urged on the trustees to provide better nursing arrangements, but they have hesitated to incur the necessary expense. Action cannot be deferred much longer if they would avoid damaging comparisons. A suitable area for future extensions must be provided somewhere in the neighbourhood. The Jubilee Home, Wanganui, does not yet appear to satisfy the aspirations of the Board who have found it difficult to find suitable officers to improve its management. The Thames Home, at Tararu, is improving. The new management and the extension of the buildings may be expected to secure greater efficiency in the future. The Whangarei Home is well looked after, and occupies a fine position. The Chairman looks after the management very carefully. At Gisborne the cottages and garden are a mere makeshift, but the inmates are better looked after than formerly. At Hamilton the aged poor are lodged in a good-sized cottage within the Hospital grounds, but they have hitherto for the most part been left to look after each other. A master and his wife have been appointed, who should be capable of looking after the cooking and help the feebler inmates. The Napier Board have at length taken steps to remove the reproach which they have so long deserved for their neglect to provide a suitable home for their poor. A fine piece of land has been secured within a reasonable distance of the town, and a convenient plan has been considered. The Home at Nelson is well looked after, but the building is the old immigration barracks, and cannot be allowed much longer to serve for its present purpose. A new home in a suitable situation ought to be provided. At Timaru a new home is required. A suitable site was procured, but after being approved by me it has been diverted to form a site for a hospital for infectious diseases. At Oamaru the old building ought to be removed ; the plan of the new home ought to be completed. I found the inmates were kindly treated and contented. The Southland Charitable Aid Board ;ire vigilant and careful in their inquiries when dealing with applicants for out-door relief. The result is that, whereas in 1898 the expenditure in Southland upon out-door relief was £1,900, in 1904 the amount had been reduced to £954.

Dr. Thomson must be admitted to be attentive and capable. The new Matron, Miss McLeod, trained at Invercargill, promises well, and had everything in the best possible order. The drainage which was left from a point nearly half-way down the slope of a bank to find its way into a small sluggish creek about a mile or more above where it falls into the river has, as I hear from Dr. Thomson, been rectified since my visit. The trustees take a deep interest in the Hospital, and it is well managed.

A great deal of uneasy feeling prevailed during the year in consequence of certain unfavourable expressions in last year's report. At a meeting with the Chairman and another member of the Board, they expressed themselves as being anxious to get from me detailed reasons for my dissatisfaction. It was apparent to me that they knew all about it, and only wished that I should bell the cat, which I did in the plainest terms. It had been apparent to myself and Mrs. Neill for a considerable time that there was little or no discipline in the Hospital, owing to the extreme kindness, not to say softness, of the Matron. The nurses did as they liked, and irregularities of a grave nature had occurred. The Matron was, and is, extremely popular, and the Board shrank from interfering.

It has been a year of great turmoil and confusion. At length a Royal Commission was appointed, who presented a very drastic report. One good thing has issued from it. The honarary staff has been, with the exception alluded to, reorganized, and Dr. Roberton has been appointed Chairman of the staff. He is determined to do all that in him lies to effect reforms. Of late years the proper training of nurses has been almost entirely neglected. lam glad that Dr. McKellar, to whom the nurses in the past have owed so much, has resumed teaching, and I have no doubt the State examination will in future show great improvements. The Board are very much in earnest in their determination to reform the sanitary arrangements, which have been so often pressed on their notice. The bath-rooms, closets, and urinals are all out of date. The whole space in the corner towers ought to be gutted and the utterly useless back stairs removed. Hot- and cold-water basins should be provided in each of the ward and the whole ground floor renovated and ventilated as I have recommended.

It would serve no good purpose to detail the step by which the Hospital Doctor was harassed by the Board, how they got into a tangle over the plans for the new Hospital. More than once they have appointed unsuitable Matrons, while they rejected first-rate applicants. It was so hopeless at one time that I offered, subject to the Minister's approval, to administer both the Wairau and Picton Hospitals for some years in the belief that I could save enough money to build a new Hospital. The situation had become so intolerable that Dr. Anderson left the district. Dr. Bennet, a good man, has been appointed, and I hope quieter times will supervene.

This is only the smooth surface of stagnation, allowing an accumulation of weeds to check progress. Such has been the condition of Christehurch'Hospital for the last few years, but the present Board have had details of inefficient internal management placed before them in plain terms by the honorary staff and this by Department, and seem willing to do what lies in their power to remedy the errors of former Boards.

The nursing staff consists of a Matron, another registered nurse, and a probationer. The probationer understood that she could not be trained here. There was no hot-water system, and no sinks in connection with wards and dispensary. There was no telephone. Sanitary matters are under consideration, and some alterations are pending. At the commencement of the year the Department learned through the Press that the Board had issued by-laws framed on those of Northern Wairoa adversely criticized in the last report. We have no official notice of these, and if the facts are as stated the action of the Board is illegal. According to section 63 of the Act of 1885, " A copy of every by-law when made shall be sent to the Colonial Secretary before the same shall come into operation."

Visited 12th February, 1905, I found a great deal of discontent about this Hospital in the district. I have always regretted that the Board did not continue to pursue the policy of having a properly trained nurse as Matron. The appointment of Mr. and Mrs. McLinchy I consider a retrograde step, though the wife is a good, hardworking woman. The people of the district take a great interest in all that concerns the Hospital. I have found four cases of undoubted typhoid in the Hospital, and I am bound to say that all concerned did their very best to look after them.

The controversy about the wardsmen has apparently been adjusted. The patients had no complaints. The beef-tea was tasted and found to be of good quality. The newer wards are well planned and constructed. Miss Fraser is pleased to be back to her work again after a visit to hospitals in America and the Old Country. She reports that the staff is working harmoniously. The dental school will add a speciality to the Hospital. It seems to me that greater care should be exercised in the selection of probation nurses for this Hospital, and that individual members of the medical staff should no longer give certificates of competence to nurses on leaving. The Hospital certificate signed by the Chairman alone should have any value for trustees of other hospitals in search of nurses or matrons.

The plastering, however, as is so often the case, is execrable. Insufficiently dried timber has been used, and innumerable cracks develop in all directions. I was delighted to find the Matron, Miss Thompson, a well-trained and certificated nurse, in charge. Dr. Hyde is an able and vigorous man who looks thoroughly well after the Hospital. This district for many years has owed a great debt of gratitude to Mr. Stevens for his untiring devotion to the interests of the Hospital. I was very pleased with the state of the institution.

It contained sixteen men and twenty-four women on the day of my visit, and they all spoke in the highest terms of the treatment they received. I found no indications of a demand for outside doctors to be allowed to attend their own patients in the Hospital. A great deal of credit is due to the Chairman, Mr. Nolan, for the excellent condition in which I found this institution, and especially for the tact and firmness with which he resisted in former years the above preposterous demand. Dr. Morrison is in all respects deserving of confidence, and the Matron could not be excelled.

Visited 29th July, 1904. —Nothing has as yet been done to finish the new Hospital. Probably the unsatisfactory way in which the new wing was finished, especially as regards the plastering, has something to do with this. In any case, it is time to proceed to completion so as to enable the new buildings to be applied to their proper use. I found everything in good order, and got good accounts of the Matron, Miss Fothergill. The medical and surgical work is well attended to by Dr. Morice. There is the usual overwhelming proportion of chronic cases, and this makes it difficult to secure or retain capable nurses and matrons who are anxious to improve themselves.

There were at the time of the visit fourteen patients, four of whom were convalescent. I have always regarded this Hospital as unnecessary, being within easy reach of New Plymouth and Patea, or even Wanganui. The New Plymouth Board could easily have resisted the demand that arose in the district for a fully equipped hospital of their own by providing a small six-roomed cottage, properly looked after. For precisely a similar reason an unreasonable demand has arisen for a hospital up to modern requirements at Stratford. This is not required at present. A cottage hospital would have been ample for all demands. I would earnestly appeal to those responsible to consider the ratepayers.

Following the example of Greymouth, where some years ago an instalment of a new hospital designed on the same lines as the Wanganui Hospital, and of Westport, where a fine new hospital has already been planned, the people of Hokitika have raised an agitation for a similar hospital in their district. The Matron, Miss Griffith, an excellent, well-trained nurse, has everything in good order. But capable nurses, anxious to enlarge and improve their experience can, with difficulty, be retained, because all the Westland hospitals are so largely occupied by chronics, from nursing whom so little is to be learned. Dr. Teichelmann's skill and attention are well known.

Some alteration in the vicinity of the kitchen have been approved, and a water-carriage system of drainage is under consideration. The nurses in this Hospital are nowadays so well trained that I find them in responsible positions in other hospitals, and lam always glad to see them there. The improvements made last year have made the building much more presentable. At my visit I expressed the hope that there should be no more tinkering with the kitchen accessories until such time as the present position of the boiler and other requisite changes could be undertaken in a comprehensive way.

Dr. Sutherland is contriving to set aside a room for operations, which is not to be dignified by the name of operating-room. This Hospital serves a somewhat exceptional district. Tapanui patients can as readily be taken to Dunedin as here, and, as a matter of fact, are. Tapanui, some years ago, wished to set up a separate institution ; but this was successfully resisted by the Tuapeka Board, who feel that they could not agree even to put up a branch hospital there while denying the like to Roxburgh and Waipori, whose claims would inevitably be pushed, when, if logical in their concessions, they must exceed their funds.

A new hospital has been a sanitary necessity for many years since I first condemned the old one. The conduct of the Board has become to me incomprehensible. Their frequent changes of policy and of proposed sites for the new hospital are bewildering. They introduced to the contagious-diseases annexe outside practitioners of the opposite faction, who asked payment for their services. Dr. Butement, after years of good service, has been removed.

There were eight patients on the day of my visit. Dr. McKnight is a thoroughly reliable man, and Mrs. and Mr. Byrne second his efforts in the most praiseworthy manner. All the patients spoke in the highest terms of the universal kindness with which they were treated. Everything was in perfect order. A fine supply of new instruments had just arrived.

Visited 3rd December, 1904, and 23rd April, 1905.—1n Dr. Mackay's absence on leave, I found this Hospital in carge of Dr. Bett. Since Dr. Mackay's return, lam glad to find that the uneasy feeling which I found prevailing at the time of my visit has largely disappeared. The difficulty about the new Matron's registration has been removed, and I hope that more peaceful times are coming. As regards its internal condition, the Hospital is well managed, and I found everything in good order.

Visited 24th August and 24th December, 1904.—Since the advent of Dr. Leatham, this has been one of the best-managed Hospitals in New Zealand. The record of operations is remarkable, and there is no question of the skill and care with which they are carried out. Miss Brown, the Matron, is very efficient, and the whole institution is in a most satisfactory condition.

The V-jointed lining runs horizontally, which, while serving to brace the building, affords a good lodgment area for dust and all that it contains. There is no closet in the female side, and the pans have to be taken past the operating-room door to the staff closet. Dr. Horton told me during the visit that he did not consider it compatible with his professional dignity to continue to serve the trustees under the existing by-laws. He has resigned, and has been replaced by Dr. Collins, late of the Auckland Hospital.

Visited 20th February, 1905.—1 found this Hospital in a condition creditable to all concerned. The Chairman, Mr. Mainland, has for many years devoted himself in the most praiseworthy manner to looking after all its concerns. A most amicable spirit pervades the relations of the leading medical men of the town. They take the Hospital in turns each year, and there is no hospital in the country where fuller advantage is reaped by the community. Mrs. Desmond, the Matron, is kindly and attentive. She is assisted by five nurses, two of whom are certificated. The total number of beds is thirtyfive. The beautiful sunny verandah runs the whole length of the new ward, and some of the patients live in the open air. There was nothing to find fault with.

OTAKI HOSPITAL. It is, however, still lighted by kerosene. The male-ward is very good and suitably furnished. A strip of linoleum 6 ft. wide is laid along the middle. The sanitary appliances are good. There is an isolation-ward with two beds. The female-ward has two patients and has five beds and a cot. The nurses' accommodation and the kitchen are suitable. There is a beautiful verandah, with both ends closed in by glass. A former patient, Mr. Williams, left about £3,000 to the Hospital.

The numbers have gone up, and the question of the addition of a large ward has been discussed with Messrs. Luxford, Essex, Dr. Martin, and the architect. As a result of the crowding, the quarters for the staff, consisting of the Matron (Miss Dougherty), four registered nurses, and four probationers, have become inadequate. It is proposed to renovate and adapt the fever cottage for a mlrse's home, and to build an infectious-diseases block on an addition to the estate newly acquired.

I cannot appraise the blame for this state of things, and can only deplore the spirit, which for many years permeated the whole district. The Picton Hospital, taken by itself, is well managed, and is well looked after by Dr. Redman. The Chairman, Mr. Fell, spares no pains in looking after its interests, and the patients are very kindly treated.

Through curiously involved local interests I founfl it impossible to induce the authorities to do the right thing, so that now the position of the hospital at Frankton has become a cause of wonder to every intelligent passer-by. Since the new Matron, Mrs. Judge, has been placed in charge I find a great improvement in the internal condition of the institution. As for the care and well-proved skill with which Dr. Douglas does his work nothing could be more satisfactory.

Here, as everywhere else where I attempted to stem the current of the extravagance of local bodies, I was defeated. It is true that the Orepuki District and now the progress of settlement beyond the Waiau lend more justification for its existence. The practice of the Hospital has become much more enterprising in the hands of Dr. Trotter. The nursing is very good.

Unfortunately, however, the additions were built of unseasoned timber, so that the plastering, originally inferior, is now full of cracks, and the workmanship as a whole is not such as ought to be seen in a modern hospital. Another feature in the management of this Hospital I must energetically protest against, and that is the utterly unjustifiable system they have adopted of allowing the medicaljmen to send their own patients into the hospital, and to continue their attendance on them as patients} there. Dr. Unwin seems to me an eminently suitable man, extremely conciliatory in disposition, and|as likely a man as I ever saw to work such an anomalous system satisfactorily; but I hope the medical men of the town will be content to act as an honorary staff, and put an end to the present state of affairs. Notwithstanding all this I have nothing but commendation to offer regarding the internal condition and general administration of the Hospital.

The Board seem to have experienced some difficulty about successfully preparing their probationers for the State examination. They find that their training is necessarily inadequate, chiefly because the doctors cannot be expected to take the trouble required, though the new Matron may do so. It is impossible to complete their preparations for examination. The only remedy seems to be that this and other hospitals similarly situated should employ only nurses already certificated, or at least such as are ready for their final examination, by virtue of a training received in a larger hospital.

Visited 11th January, 1905, and 24th May, 1905. —The old Hospital in this place was built without any rational plan, and cannot now be added to, and, besides, the white pine which entered so largely into its construction is so drilled by the borer that it has become unsafe. A fine new Hospital has been designed by Mr. Atkins, of Wanganui, who made so good a job of the Hospital there, and I am confident he will do this equally well. As regards the actual working of this institution, I found everything in a very satisfactory state, reflecting great credit on Dr. Douglass and his staff. There were forty-six patients on the day of my visit. The Matron, Miss Rothwell, is assisted by a staff of nine nurses. Very large ideas prevail in this district as to the completeness of the Hospital organization required.

I found it in good order. The Matron, Mrs. Chapman, has done good service for many years, but the time has now come when she should be retired on the most generous possible terms, and replaced by the best certificated nurse that can be found. It is almost too much to expect that friction will not arise between a Matron of Mrs. Chapman's age and certificated nurses under her command. That is a position of unstable eqiulibrium and full of difficulty.

Visited 12th April, 1905. —At the period of transition, the new Hospital being prepared for the reception of patients from the old one, there were three patients. The new Hospital shows some structural defects ; gaps were left exposing scrim near door-frames and skirting-boards owing to timber shrinking. The position of the door of the room originally intended to be used for operations would not allow of a stretcher being carried in. The baths are boxed in. Dr. Swanseger, who has been the Medical Officer since October, is doing good work.

The new ward, containing twenty-four beds is, I think, unnecessarily large, but well finished. There is a bow window projecting into the ward, which loses a bed, and the ceiling of the commodious linen-room at the back is horribly discoloured. The lavatory block attached to this ward is not cross-ventilated, and schemes for remedying this were discussed with the Chairman. A great deal of interest is taken in the welfare of the Hospital, and it is well administered.

The opening of a fine home for the nursing staff and of special wards for the care of chronic invalids, and the proposed erection of shelter for consumptives, have increased the responsibilities of the Trustees to an extent that they as yet little realise. Dr. Ewart's field of work is greatly extended, and it is well that he now has an assistant medical officer. Miss Payne's return to her former position of Matron gave satisfaction to every one interested in the management of the Hospital. Sister Jean has been promoted to the charge of the chronic invalids wards. There is always a long list of applicants for the post of probationer in Wellington Hospital, owing to the excellent training to be received under Dr. Ewart and Miss Payne. The structural defects of the sanitary arrangements at the rear of the wards ought, as soon as possible, to receive serious consideration at the hands of the Trustees. If the cost of providing accommodation for the nurses as at the Wellington Hospital (caused by the eight hours system) he emulated elsewhere our expenditure will go up by leaps and bounds.

There are now 35 (male 28, female 7), making with 16 old men and old one woman a total of 52. The Hospital is an unsuitable building, and has been added to and altered so that there is no possibility of a good plan. The proposal to spend £9,000 or £10,000 I had condemned. The site was inadequate in area. On the assurance, however, of a deputation, headed by the late and the present Chairman, that a space of 5 acres had been provided, and that this would be filled in with sand, &c, and properly planted and fenced, I agreed to waive my objections. The cost mentioned is excessive
