1904. NEW ZEALAND.

HOSPITALS AND CHARITABLE INSTITUTIONS OF THE COLONY (REPORT ON THE), BY THE INSPECTOR OF HOSPITALS.

Presented to both Houses of the General Assembly by Command of His Excellency.

The Inspector of Hospitals and Charitable Institutions to the Hon. the Minister for Public Works. Sir,— Charitable Aid. 27th July, 1904. The expenditure on charitable aid during the year ending the 31st March, 1904, was— Indoor relief, £46,613 ; outdoor relief, £42,618 : making a total of £89,231. This is apparently a reduction on last year's total which stood at £94,158, but, as £7,235 was spent in buildings and purchases of land, the more exact comparison is with a total of £86,923. The large sums spent on charitable aid, especially on outdoor relief, of recent years calls for special notice, and its growth is a matter for serious consideration. How far the distributors are to blame it is not easy to estimate, but that they are to blame is writ large when one compares the amount of outdoor relief given in different centres of more or less equal population. It may be that they are careless or facile, or lacking in system; it may be that humanitarianism outweighs wisdom in councils where these factors should counterpoise ; but it requires little penetration to discover that the trustees of public money do not accept the trust with equal seriousness. Comparing the state of things in the four larger towns we find :— 903. 1904. The Wellington and Wairarapa "District altogether spent £6,134 on outdoor relief, made up as follows : Wellington Benevolent Trust, £3,887 ; Wellington and Wairarapa Charitable Aid Board, £1,527 (this is money given to counties, &c, to expend) ; Masterton Benevolent Society, £407 ; Wellington Ladies' Christian Association, £313. A family or individual can be helped to tide over a time of special trouble without the loss of self-reliance or self-respect; it is the habitual applicant for aid who should be checked by being made in some form or other to work out the relief given to him or her. Some Boards have on their books recipients of charitable aid to the third generation, showing that heredity in these matters comes into play. The payment of house-rent for applicants—unless as quite a temporary matter—is an economic evil. Such an expenditure of public money is prejudicial to the recipient's next-door neighbour, whose rent is a little behind, but who will pay in the long-run ; for so far as the landlord is concerned, the recipient of charitable aid becomes the better tenant. tion for the neighbour and others in the same district to cease struggling for honest independence is too obvious. Laxity in the administration of charitable aid breeds poverty, and once more I would point out that the remedy lies to a very great extent in the hands of the citizens themselves through their representatives on local administrative bodies. Begarding the institutions designed to provide indoor relief for the destitute and aged, I have to report as follows : The Old People's Home, New Plymouth ; the Ohiro Home, Wellington ; the Jubilee Home, Christchurch; Tuarangi, Ashburton; and the Lome Farm, Southland, all merit high praise for efficiency, comfort, cleanliness, and economy. The only drawback to the Ohiro Home is its site. The Costley Home, Auckland, is busy making reforms, and I hope next year to be able to report on satisfactory conditions of cleanliness and comfort. The Caversham Benevolent Institution remains understaffed; as has been often pointed out the whole institution requires for its thorough-going reform removal to some larger ground-space in the country. The Jubilee Home, Wanganui, is fairly satisfactory, and the same may be said of the Old Men's Home, Tararu. The Whangarei Home is satisfactory, and the Gisborne Home better than formerly. At Hamilton there should be a resident caretaker. The Napier Old Men's Home is a disgrace to the Board, and something should speedily be done to remedy matters. Notes made on the visit of an Inspector in June, 1904, say, "The buildings are old, dilapidated, unlined, infested with bugs. The bedsteads are narrow, no sheets or pillowcases, bedding dirty, and pillows in some cases filthy. Some of the inmates were very dirty, and are either neglected or beyond control." Nekon Old Men's Home is an old building, bug-infested, kept clean and habitable by indefatigable labour on the part of the matron, Mrs. Matthews. The Home at Timaru is also an old building, but the site has been chosen for a new one. At the Oamaru Home there is one good ward; the rest of the building is very unsatisfactory, although it is well managed and as clean and comfortable as such a building can be kept. Other institutions that have been visited during the year are St. Andrew's Orphanage at Motueka, thoroughly satisfactory in its management under Mr. and Mrs. Haycock. The children looked strong and healthy and find here a real home. St. Mary's Home, Wellington, and St. Mary's Home, Christchurch: In both these institutions the number of inmates is diminishing. The Linwood Refuge is still doing admirable maternity work. The Samaritan Home, Christchurch, is anxious to be placed on a firmer legal basis in regard to its inmates—the dregs of society. The patience and kindness of the Matron, Miss Early, show that her heart is in the helpful work. The Alexandra Home for Friendless Women, and the Levin Memorial Cottage Home for Children, Wellington, are working well; also the Convalescent Home, an invaluable haven of rest for delicate and convalescing patients. Hospitals. Generally speaking the management of our hospitals is satisfactory. Faults of construction in new hospitals are many, but this cannot be remedied until Hospital Boards are legally compelled to submit their plans for approval to this office before commencing to build. Eatepayers would be astonished if they realised the amount of money simply thrown away on defective material and workmanship, as well as the faults of construction that necessitate alteration when the building is supposed to be complete, in order to comply with laws of sanitation. I would draw your attention to methods in use at the Northern Wairoa Hospital, Aratapu, that are a menace to our hospital system. Any person who pays £1 6s. a year is entitled to the benefit of free medicines and attendance by the doctor for himself, his wife, and children. If this benefit contributor or any of his family becomes an in-patient of the hospital they receive all the benefits thereof for 15s. a week.' This means that the Aratapu Hospital is turned into a sort of private benefit club for the neighbourhood. An exceptional concession on the same lines was made to local committees in the isolated district of Mercury Bay, and this concession has been copied and taken advantage of by the Northern Wairoa Hospital, which cannot urge the same plea of isolation or difficulty of access. I append a table giving a few comparative facts respecting our four largest hospitals :—

There are now 391 nurses' names on the Register. Examinations were held at different centres on the first Tuesday and Wednesday in May and December. At the two examinations fifty nurses passed the preliminary examination in anatomy and physiology, and sixty nurses passed their final and practical, and were added to the list of registered nurses.

Dr. Copland (Gore) and Dr. Crosby (Christchurch) set the papers on anatomy and physiology, Dr. Pabst (Auckland) and Dr. Talboi (Christchurch) set those on nursing. Drs. Hope Lewis, Perkins, Eoberts, Eeid, Andrew, and Godfray, together with Misses Eothwell, Palmer, Collins, Ewart, Maude, Harkness, and Dougherty were examiners in practical work in May ; Drs. Williams, Colquhoun, Acland, Young, Boyd, Andrew, Beed, Christies, Henley, and Roberts, together with Misses Morrison, Ewart, Maude, Godfrey, Berry, Dougherty, and Payne, and Mesdames Irving and Holgate, were examiners in practical work in December. Expenses connected with examinations (examiners' fees and travelling-expenses) amounted to £104 14s. 10d.; fees received from nurse candidates amounted to £88 7s.

The Hospital was badly constructed, and was originally built of old timber containing vermin. This evil has grown and multiplied until it has become imperatively necessary that the old portion be destroyed by fire. A grant of £1,300 has been promised by Government towards the erection of an entirely new building, and it is to be hoped that the Board will see their way to make a start without further delay. Dr. Anderson and Miss Wyatt deserve credit for working well under great difficulties.

It has been found necessary to increase the accommodation of the nurses' home. A new steam-disinfector has been installed. Notwithstanding the many advantages conferred on Dunedin Hospital by connection with a medical school, there are some drawbacks, and one of these I find is that the students do so much of the surgical dressings and other work in the wards that I fear the nurses have not the same advantages as the nurses in other training-hospitals.

Since then, Miss Griffith, a first-class Christchurch nurse, has been appointed Matron, and will greatly improve matters in regard to nursing and discipline. There are no separate lavatory arrangements for the staff and no proper hot-water supply for baths.

Dr. Mackie has been granted leave of absence on account of health, and Dr. Betts is acting in his place. Miss Kohn was appointed Matron last September, and has much improved the nursing and.discipline. The small destructor employed seems to be an inexpensive as well as an efficient method of destroying refuse of all sorts. The operating-room is.not well lighted, neither is it supplied with hot water, also the closet in the immediate neighbourhood of the operating-room should be at once removed.

The Hospital is evidently well managed, and some excellent work is done by Dr. Leatham and his colleagues. A water-supply with sufficient pressure in case of fire is to be introduced shortly. The situation of this Hospital is excellent, and an annexe is in contemplation for the open-air treatment of phthisis.

Owing to the unavoidable overcrowding it has been a trying year for Dr. Ewart and the staff. The Nurses' Home is almost ready for occupation, and this will free the two wards hitherto used as nurses' dormitories, &c, for the use of patients. Whilst alterations are in progress it is to be hoped that the Trustees will take in hand some much-needed modern improvements in the bath-rooms and lavatories attached to the older wards.

Miss Stewart, the Matron, is now a registered nurse. The Hospital was in very good order. I pointed out to the Chairman a very serious structural defect. There are steps intervening between the main building and the kitchen, and up these steps nurses have to carry meals backwards and forwards to the wards. The Chairman promised that this defect should be rectified. I have, &c, D.
