Return of Fatal Accidents in connection with Machinery during the Financial Year ended 31st March, 1916.

Description of Machinery. Name and Age of Person injured. Date of Accident and Nature of Injury. Cause of Accident, and Remarks. Name and Address cf Owner. The Milburn Lime and Cement Company (Limited), Dunedin Shafting James Duffey ; 35 years 5th May, 1915 ; fatal .. It is presumed that Duffey was greasing a ball-wheel, and while reaching over to screw down a grease-cup he fell on to the shaft and was whirled round the spindle with tremendous speed. His death must have been instantaneous. George Pinnock, Wellington .. Electric lift George Pinnock ; 50 years 22nd May, 1915 ; fatal .. .. | Pinnock put his head through an aperture on the first floor, not knowing that the cage of the lift was coming down. He called to some one below, when the descending lift caught him and crushed his head between the partition and the cage, causing instant death. W. N. Burcher, Havelock North Oil-engine W. W. Foster ; 62 years 1st September, 1915; fatal Foster was sawing wood, when the belt came off. He went to stop the engine and was caught in the loose belting, when he was hurled against a wood-pile. His death took pace two days afterwards. Joseph Liggins, Tokomaru Flax-mill W. A. McLeod ; 22 years 7th December, 1915; fatal ■ McLeod was engaged oiling the bearings of the shaft, when his clothing was caught on the collar-stud of the shaft. He was drawn around the shaft and crushed with such force as to cause his death. Hydro-extractor .. Robert Russell, 51 years Nelson Bros. (Limited), Gisborne 20th December, 1915; fatal The machine burst into fragments while in motion, and one oi more of the parts struck Russell, causing instantaneous death. Russell was attending to the machine, being in charge at the time.
