In his report of the inquiry into the accident on the London, Midland and Scottish Railway at Watford on November 9, in which five men were killed and one injured, Mr J. P. S. Main, assistant chief inspecting officer of railways, said that the primary cause was due to Robert Bolton, the look out man. Bolton, who failed to warn the men of the approach of the train, had since been' unable to work through shock. He had* an excellent record, and his lapse was inexplicable.
