WAR GRATUITY
FORMS OF APPLICATION ISSUE BEGUN IN DUNEDIK Post Offices throughout the Dominion began the issue on Thursday of forms ot application- for the. war service gratuity. They are of two types, one for men discharged \froni the forces and the other for widows or next of kin of deceased service men or service women. There has been a steady demand at the Chief Post Office for these, several hundred having already been distributed. '.-■■'. On the form issued to discharged service men, the applicant must state his full name, the service, or services, 0f... which he. was .a„niember, .his .rank on discharge, whether he is in receipt of a war pension and whether .this is permanent, for total or partial disablement, or temporary. Space is provided for details of fully-mobilised, service in the Army, Navy, or Air Force. These details include the applicant's service number, the dates from and to which he served, both in New Zealand and overseas, and, in the case of Army service within the Domipion, his unit and military headquarters. On the back of the form, which, on completion, is to be handed" to the nearest post office for .transmission'to Base Records, are instructions to the applicant and a list of post offices at which gratuity accounts. may. The applicant must nominate the office in which his gratuity is to be deposited. - An applicant in respect of a deceased service man or service woman must give his, or her, full name, the seN vice number, surname, Christian names and rank of the service man or service woman, the date of death, and the service to which deceased was attached at the time of, or prior to, his or her death, and must state whether this occurred in New Zealand or overseas. OTHER QUESTIONS. Other questions to be answered are whether deceased was married or not, or a widower, divorced, or separated from his wife; what relationship he was to the applicant; whether - the applicant applies for himself, or herself only, or for the children of deceased only, or for both the applicant and the children, the names of whom are to be given on the hack of the form. Information is also sought as t<j whether the service man or service woman left a will and who are the beneficiaries under it. If the applicant was dependent on deceased, particu lars as to the weekly amount received as allotment or allowance are to be given. The application is in the form of a statutory declaration, to be_made before a justice of the peace, solicitor, post master, or other person appointed to take declarations. On completion, it must be handed in »to the nearest post office, or posted to the Director of Base Records, Wellington. A pamphlet giving information concerning the gratuity accompanies each form of application. .
