The Transport Ministry's official re- . port on the rail crash near Taunton on November 4, in which 27 persons were ;killed, states: "The sole cause was an unaccountable lapse on the part of the driver. His breakdown may be partly attributed to the operating conditions in a black-out and to the general strain «.which railway servants, in..' common with other members of the commun4ty, are undergoing at the present time." ■.'.,.,
