Report Finds Pilot’s Throttle Failed

(N.Z. Press Association) WELLINGTON, Sept. 21. Material failure of the throttle bell-crank was the probable cause of a Cessna 188 Agwaggon crashing while spreading superphosphate at Akau, in North Taranaki, on February 28 this year. This was the finding in a report issued by the air accidents inspectorate of the Ministry of Transport. The pilot, Frederick Ernest Ewings, died in the crash.

The report said that before its last take-off the aircraft had completed two sevenminute flights without incident

Investigation of the crashed aircraft showed that as far as engine control was concerned the sole defect was the throttle bell-crank which showed evidence of failure through fatigue. Failure of the throttle bellcrank in flight would have resulted in immediate and irretrievable loss of power.

During the turn towards a forced landing, the aircraft stalled and entered an incipient spin which could not be arrested before the aircraft hit the ground.
